• Ensure the proposed member understands the expectations and responsibilities of Rotary membership.
• We highly recommend the proposed member attend an orientation meeting prior to submission.

Once submitted, this form will be reviewed by the Membership Committee. If the proposed member is accepted, this application will be retained for as long as they remain an active member.
